If you're worried about her not having an orgasm (which you should be if you value your sexual relationship), then you will want to focus on her clitoris.  If she continues to have pain with sexual intercourse, you can always bring her to orgasm by means of oral sex and simply masturbating her to climax.  Some women have "hymens of steel" that refuse to break and can consequently make them experience pain during sexual activities.  Add spice to your sex life by trying a new sex position each time you have sexual intercourse.

Playing fantasy games of what you would like to do rather than give up like “normal” people (wherein “normal” people are the ones who give up because of back pain. We are sexual beings, throughout the lifespan.  This also includes the later years of life, which are often overlooked in discussions of sexuality.

In terms of maximizing sexual pleasure, it is very important to stress that all you really need is your imagination and the willingness to experiment to open up new areas of intimacy.  People who care about their sexual satisfaction and about their partner's satisfaction will get the most benefit by being open to each other.  Nonetheless, you and your partner need to find a way to discuss your back pain, and how it will affect-—or already does affect—the sexual component of your relationship.  Maybe a little planning before hand would help. Factors such as emotional connectedness, a bouquet of flowers sent for no reason, attentive listening, saying thank you for the little things, or sending the kids to Grandma's for the night can all add to sexual satisfaction. Make sure that the phone is unplugged (even the answer machine is disconnected), the washing machine/dryer is silent. Anything that could disturb your ‘peace’, pay attention to detail and you will not be disappointed.
